Enroll in the Optional Retirement Program Online

Online enrollment is the fastest and easiest way to enroll. Before you enroll, have the following information available:
     1.  Your Social Security number
     2.  Your beneficiary’s Social Security number, birth date and address
     3.  Your investment allocations (included with each vendor's packet)

To complete your enrollment, please send the following forms to the University Benefits and Retirement Office: 

1.  The University of Tennessee Optional Retirement Program "Premium Distribution Specification" form.

2.  A copy of your printed confirmation page from your vendors website OR confirmation number written of the UT Premium Distribution Specification form.

2.  Your notarized copy of your "Notice of Election to Participate in the Optional Retirement Program" NOT REQUIRED AFTER 7/1/2014
