1. Your Social Security number
2. Your beneficiary’s Social Security number, birth date and address
3. Your investment allocations (included with each vendor's packet)

To complete your enrollment, please send the following forms to the University
Benefits and Retirement Office:
1. A copy of your printed confirmation page from your vendors website
2. Your notarized copy of your "Notice of Election to Participate in the Optional Retirement Program"
3. The University of Tennessee Optional Retirement Program "Premium Distribution Specification" form.
